Theoretically that can happen too.
To eliminate this problem... when you grab the grenade place your finger or thumb over the plug... then pull the pin... if you are not careful you can dislodge the plug as you pull the pin...making sure you have a finger or thumb on it ensures it is well seated ...

Trust me.. we have had every possible "accident" with these things..

When they do anything unexpected.. it can typically be attributed to improper priming or deployment by the user.
