When using the Airsoft Surgeon Loading Nozzle with Nine Ball Dyna Piston Head, I like to use the smaller o-ring that comes with the 9Ball Dyna.
As for the BB being pushed out by the slide's increased return force, I've found that the Nine Ball hop up rubber to be a great solution for holding the BB in.
